- Legalized Diploma
- 4 (four) signed and stamped declaration letters
- Colored Photos: 3x4 = 5 sheets, 4x6 = 5 sheets
- Photocopy of both parents’ ID cards (KTP)
- Photocopy of Family Card (KK)
- Photocopy of Birth Certificate
- Photocopy of previous school grade report card
- Medical tests: Chest X-ray, drug-free, and Hepatitis B test results
